His helmet has flown off. There is extensive bruising around the head.<\/p>\n<p>On arrival, he appears unconscious.<\/p>\n<p>You start with an ABCDE approach. You pay particular attention to A because you know that in patients with reduced consciousness, the airway can be at risk (because of the loss of muscle tone leading to a floppy airway, as well as the tongue falling backwards). Think of a very drunk person (i.e. someone with reduced consciousness) deeply snoring. Think of the folds of fat and soft tissue vibrating in that floppy airway. Now imagine that airway getting even more floppy to the point where it occludes. A GCS of around 8 or lower is traditionally taught as when you would get support for the airway.<\/p>\n<p>B and C are fine, so you move on to &#8216;D&#8217;. This stands for disability.<\/p>\n<p><strong>Why are we assessing the patient&#8217;s disability\/handicap at this stage?\u00a0<\/strong><\/p>\n<p>Although the word &#8216;disability&#8217; is used, I prefer to think of it as neurological &#8216;deficit&#8217;. To cause reduced consciousness, there must either be extensive <em>bilateral<\/em>\u00a0cortical damage (as if one hemisphere is working normally, this will still project to the reticular activating system as normal, so consciousness will be maintained), or damage in the brainstem. Alternatively, there may be a systemic problem with the blood supply to the brain e.g. hypoglycemia or a toxin.<\/p>\n<p><strong>Focal problems<\/strong><\/p>\n<p>Anything that raises intracranial pressure can cause a reduced consciousness. The commonest problems are haemorrhage and space occupying lesions such as tumours\/abscesses. Reduced consciousness can also occur after brain injury without a specific space occupying haemorrhage e.g. multiple brain contusions.<\/p>\n<p>The patient could also be post ictal after a seizure. This seizure could have been focal or generalised to begin with, but would have had to been focal with secondary generalisation or primarily generalised to cause reduced consciousness.<\/p>\n<p><strong>Raised intracranial pressure&#8230;?<\/strong><\/p>\n<p>Think persistent headache that\u00a0<em>worsens<\/em> with lying flat, bending over or coughing, vomiting (especially effortless), focal neurology and seizures. Late on, you get <a href=\"https:\/\/www.emsworld.com\/article\/10453662\/ems-recap-intracranial-pressure-and-cushing-reflex\" target=\"_blank\" rel=\"noopener\">Cushing&#8217;s reflex<\/a> and papilloedema (can you make out the optic disc margin below? Me neither. These almost always occur without impaired consciousness.<\/p>\n<p><strong>Systemic<\/strong><\/p>\n<p>Now we&#8217;re talking (unless your GCS &#8211; V is less than 3). These can be split into <strong>exogenous<\/strong> problems (drugs) and <strong>endogenous<\/strong> (toxins and metabolic abnormalities):<\/p>\n<p><strong>Endogenous<\/strong><\/p>\n<ul class=\"\">\n<li id=\"med-9780190244927-chapter-1-item2-65\">\n<p id=\"med-9780190244927-chapter-1-p-308\" class=\" \"><strong>Hypoglycemia<\/strong> (and sometimes hyperglycemia, if <a href=\"https:\/\/www.diabetes.org.uk\/guide-to-diabetes\/complications\/hyperosmolar_hyperglycaemic_state_hhs\">HHS<\/a> has developed for example)<\/p>\n<\/li>\n<li id=\"med-9780190244927-chapter-1-item2-67\">\n<p id=\"med-9780190244927-chapter-1-p-310\" class=\" \">Hyponatremia<\/p>\n<\/li>\n<li id=\"med-9780190244927-chapter-1-item2-68\">\n<p id=\"med-9780190244927-chapter-1-p-311\" class=\" \">Hypo\/ernatremia<\/p>\n<\/li>\n<li id=\"med-9780190244927-chapter-1-item2-70\">\n<p id=\"med-9780190244927-chapter-1-p-313\" class=\" \">Hyper\/pocalcemia<\/p>\n<\/li>\n<li id=\"med-9780190244927-chapter-1-item2-71\">\n<p id=\"med-9780190244927-chapter-1-p-314\" class=\" \">Severe hypothyroidism (&#8220;myxodema coma&#8221;)<\/p>\n<\/li>\n<li id=\"med-9780190244927-chapter-1-item2-72\">\n<p id=\"med-9780190244927-chapter-1-p-315\" class=\" \">Acute panhypopituitarism<\/p>\n<\/li>\n<li id=\"med-9780190244927-chapter-1-item2-73\">\n<p id=\"med-9780190244927-chapter-1-p-316\" class=\" \">Acute uremia (renal failure)<\/p>\n<\/li>\n<li id=\"med-9780190244927-chapter-1-item2-74\">\n<p id=\"med-9780190244927-chapter-1-p-317\" class=\" \">Hyperbilirubinemia (liver failure usually. High levels of ammonia can also occur in liver failure leading to hepatic encephalopathy)<\/p>\n<\/li>\n<li id=\"med-9780190244927-chapter-1-item2-75\">\n<p id=\"med-9780190244927-chapter-1-p-318\" class=\" \">Hypercapnia<\/p>\n<\/li>\n<\/ul>\n<p>A fall in cardiac output leading to reduced cerebral perfusion causes either a transient loss of consciousness until cardiac output improves, or death.<\/p>\n<p><strong>Exogenous (drugs)<\/strong><\/p>\n<p>Many drugs in overdose can cause reduced consciousness, but the main ones to bear in mind are alcohol, benzodiazepines and opiods.<\/p>\n<p><strong>Assessment of reduced consciousness<\/strong><\/p>\n<p>There are three main parts:<\/p>\n<p>a) Assess the level of consciousness<\/p>\n<p>b) Assess the pupils<\/p>\n<p>c) Look for focal neurology using signs that do not rely on consciousness<\/p>\n<p><strong>Assessing the level of consciousness\u00a0<\/strong><\/p>\n<p>This is done with the Glasgow Coma Scale. You can&#8217;t get better than this video, made under the direction of one of the authors (Teasdale) of the <a href=\"https:\/\/www.ncbi.nlm.nih.gov\/pubmed\/4136544\" target=\"_blank\" rel=\"noopener\">original 1974 paper on the GCS:<\/a><\/p>\n<p><span class=\"embed-youtube\" style=\"text-align:center; display: block;\"><iframe class='youtube-player' type='text\/html' width='700' height='394' src='https:\/\/www.youtube.com\/embed\/v6qpEQxJQO4?version=3&#038;rel=1&#038;fs=1&#038;autohide=2&#038;showsearch=0&#038;showinfo=1&#038;iv_load_policy=1&#038;wmode=transparent' allowfullscreen='true' style='border:0;'><\/iframe><\/span><\/p>\n<p>Points to remember:<\/p>\n<ul>\n<li>Motor is the most predictive component for prognosis<\/li>\n<li>You can&#8217;t score less than 3\/15<\/li>\n<li>The best response is recorded<\/li>\n<li>Decorticate posturing links with abnormal flexion, and\u00a0decerebrate links with extension (which is always seriously abnormal). This reflects if damage has occured above (decorticate) or below (decerebrate) the red nucleus.<\/li>\n<li>In other words, don&#8217;t get too hung up on decerebrate vs decorticate posturing. By the time we need to be working those sorts of things out, the patient is probably in ITU or for <a href=\"https:\/\/www.compassionindying.org.uk\/wp-content\/uploads\/2015\/02\/IN06-DNAR-forms-and-CPR-decisions.pdf\" target=\"_blank\" rel=\"noopener\">DNAR<\/a>.<\/li>\n<\/ul>\n<p>&nbsp;<\/p>\n<p><strong>Assess the pupils\u00a0<\/strong><\/p>\n<p>Have a look at his face. Did you notice anything?<\/p>\n<p>One pupil is bigger than the other. This is anisocoria.<\/p>\n<p>It can be difficult to work out which pupil is abnormal. The trick is to remember that whichever one is abnormal is either struggling to dilate or constrict. If the patient is in a bright room and one pupils <em>fails to dilate<\/em>, this is the abnormal pupil. The left pupil failed to constrict in dark light.<\/p>\n<p>The patient most likely had an uncal herniation leading to compression of the 3rd cranial nerve. The main types of brain herniation are wonderfully summarised in <a href=\"http:\/\/aibolita.com\/nervous-diseases\/47731-transtentorial-uncal-herniation.html\" target=\"_blank\" rel=\"noopener\">this link<\/a>.<\/p>\n<p><strong>Look for focal neurology using signs that do not rely on consciousness<\/strong><\/p>\n<p>Your next step of the assessment is to look for focal neurology. We can&#8217;t ask an unconscious patient to raise his arms or if he can feel everything as normal, so we rely on signs <em>that do not requires consciousness<\/em>.<\/p>\n<p>In the limbs, this means signs like tone, spasticity and reflexes.<\/p>\n<p>For the cranial nerves, this can include the doll&#8217;s eye reflex, vestibulocular reflex and corneal reflexes.<\/p>\n<p><span class=\"embed-youtube\" style=\"text-align:center; display: block;\"><iframe class='youtube-player' type='text\/html' width='700' height='394' src='https:\/\/www.youtube.com\/embed\/ze-ojg0ZqCo?version=3&#038;rel=1&#038;fs=1&#038;autohide=2&#038;showsearch=0&#038;showinfo=1&#038;iv_load_policy=1&#038;wmode=transparent' allowfullscreen='true' style='border:0;'><\/iframe><\/span><\/p>\n<p>Remember that the location of cranial nerve lesions can hint at where there may be brainstem damage. The cranial nerve nuclei (apart from I and II) all live in the brainstem.
