111 – Remember who you are

Musfasa from the Lion King speaks to you.

He asks you to remember who you are.

He asks you to specifically remember you are the renal SHO who knows nephrotic syndrome inside out:

Heavy proteinuria Spot urine showing a protein-to-creatinine ratio of > 3 to 3.5 mg protein/mg creatinine (300 to 350 mg/mmol), or 24-hour urine collection showing > 3 to 3.5 g protein
Hypoalbuminemia Serum albumin <25 g per L
Edema Clinical evidence of peripheral edema
Hyperlipidemia (not required for diagnosis) Severe hyperlipidemia

You also realise that to define AKI in the first place, changes in creatinine and/or changes in urine output are used, not changes in urea. And that increases in urea without corresponding increases in creatinine happen in GI bleeds (and to some extent in dehydration).

You are glad that this was all just a bad dream.
