301 – Not a haemolysis crisis actually

This isn’t haemolysis. The urea is crazy high. This could be a GI bleed you realise.

You scramble to begin fluid resuscitation, bloods, a VBG, crossmatching six units and reversing the warfarin. The VBG shows:

Hb 69 g/L
Lactate 3.8 mmol/L

You request the first unit of blood to be started as soon as it is available. You ask the FY1 to instead speak to the gastro consultant about the case, whilst you sort out the warfarin.

What do you do about this?

Use prothrombin complex concentrate for warfarin reversal
