Ruthless but correct

You prescribe a fixed rate intravenous insulin infusion of a soluble insulin.

You even set up appropriate monitoring of potassium, ketones and bicarbonate and keep a close eye.

You even make it clear to add 10% glucose 125ml/hr if blood glucose falls below 14 momol/L rather than adjust the fixed rate intravenous insulin infusion.

You even would have continued the long acting insulin that the patient might have been on if the patient were a known Type 1 diabetic. I did not even have to test that, I just knew you would and am giving you credit for it. Well done for being so great we can infer your greatness without directly testing you.

Score: 9/10 – everything was perfect, although the ABG caused unnecessary pain.
