230 – Smooth operator

You are so smooth with getting the tests and management organised that you decide to check up on your poor FY1 reversing the warfarin. You discover him downloading the Oxford Handbook of Medicine, 1st edition (1983) from a dodgy PDF website. He is about to give oral vitamin K 1mg and lots of green leafy vegetables for tomorrow’s lunch.

As you are about to discuss this with him, the VBG machine beeps:

Hb 69 g/L
Lactate 3.8 mmol/L

You request the first unit of blood to be started as soon as it is available. You ask the FY1 to instead speak to the gastro consultant about the case, whilst you sort out the warfarin.

What do you do about this?

Use prothrombin complex concentrate for warfarin reversal

Use prothrombin complex concentrate and IV vitamin K for reversal

Use IV vitamin K for reversal
